NOTE: The official forms are provided in Excel format only.

• FOR NEW STUDENTS

1. Download the FREE APPLICATION FORM by clicking the link below:

2. Fill in all the fields in the Application Form, offline or online. (There is no need to print out and scan.)

3. Save and submit the filled out form via email to registrar@cornerstoneacademy.edu.ph with the required credentials as attachments (in PDF, JPG or PNG formats).

4. Wait for an email response from the Registrar's Office that will tell you how to proceed.

• FOR RETURNING STUDENTS FROM THE PREVIOUS YEAR

1. Download the FREE ENROLLMENT FORM by clicking the link below:

2. Fill in all the fields in the Enrollment Form, offline or online. (There is no need to print out and scan.)

3. Save and submit the filled out Enrollment Form via email to registrar@cornerstoneacademy.edu.ph

4. Wait for an email response from the Registrar's Office.
